Roller Skate Liability Waiver
Please read and complete this waiver form before participating in roller skating activities. Your safety is our priority.
ROLLER SKATING WAIVER AND RELEASE OF LIABILITY
Location: McCloud Dance Hall, 104 Pine Street, McCloud, CA 96057
I, the undersigned participant (or parent/guardian if under 18),
acknowledge and agree to the following:
Assumption of Risk I understand that roller skating involves inherent risks, including but not limited to falls, collisions with other skaters, objects, or structures, and other hazards that could result in serious injury or death. I voluntarily choose to participate in roller skating at my own risk.
Release and Hold Harmless In consideration of being allowed to participate in roller skating at McCloud Dance Hall, I hereby release, discharge, and agree to hold harmless Kristin Cunningham, McCloud Dance Hall LLC, their owners, officers, employees, volunteers, and agents (collectively, the “Released Parties”) from any and all claims, liabilities, demands, or causes of action arising from my participation, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
Medical Authorization In the event of injury, I authorize the Released Parties to obtain necessary medical treatment for me (or my child). I agree to be financially responsible for any costs incurred.
Personal Responsibility I agree to follow all posted rules and instructions. I acknowledge that wearing protective gear (such as helmets, knee pads, elbow pads, and wrist guards) is strongly recommended.
Parental Consent (if under 18) As the parent/guardian of the minor named below, I consent to their participation and agree to the terms of this waiver on their behalf.
SKATING RULES Skate at your own risk — be aware of others and skate responsibly.
Perimeter for skating, center for dancing — please keep skating around the edges of the floor; dancing happens in the middle.
No walking across the floor — if you need to move from one end of the building to the other, please walk outside the skating area and off the dance floor.
No reckless skating — no racing, pushing, or weaving through others.
Protective gear recommended — helmets, wrist guards, elbow pads, and knee pads help keep you safe.
Supervise children — minors must stay with a parent or guardian at all times.
Skates inspected — damaged wheels or unsafe equipment are not allowed.
Follow staff instructions — any unsafe behavior may result in removal.
